All eligible employees are available to participate in the benefit plans on the 1st day of the month following the date of hire. Medical Insurance
Group medical insurance (PPO) is offered through CareFirst BlueCross BlueShield for all eligible employees that do not have other health care coverage and their dependents. There are two plans from which to choose. The 90/70 plan has a $250 in-network deductible, $15 general practitioner co-pay, and 90% co-insurance for most in-network services. The 80/60 plan has $500 in-network deductible, $20 general practitioner co-pay, and 80% co-insurance on most in-network services. All participating employees are also enrolled in the prescription drug plan. Participating employees pay a minimal premium contribution.

Dental Insurance
Group dental insurance is offered through MetLife for all eligible employees and their dependents. There are two plans from which to choose. The CorePlan offers 100% preventive care coverage, 80% for minor procedure coverage and 50% for major procedures (subject to the a deductible), and $1,000 life-time maximum for adult and children orthodontics. The High Plan offers an additional 10% coverage on minor and major procedures (subject to a deductible), as well as an additional $500 in yearly benefits. Premiums for the Core Plan are 100% paid by Smartronix and participating employees pay a minimal premium contribution for the High Plan.

Vision Insurance
Group vision insurance is offered through Vision Service Plan (VSP) for all eligible employees. Premiums are 100% paid by Smartronix. Coverage includes network benefits for eye exams as well as discounted lenses, frames, and contacts lenses.

Disability Insurance
Smartronix provides short-term and long-term disability to all eligible employees at no cost. Short-term disability may be payable at 66.67% of the pre-disability regular weekly salary following a 7 day elimination period for illness or a 0 day elimination period for injury. The maximum weekly disability benefit is $1,000 per week. Following an elimination period of 180 days of consecutive disability, long-term disability may be payable at 60% of the pre-disability regular monthly salary, with a maximum monthly benefit of $10,000.

Basic Life and Accidental Death & Dismemberment (AD&D) Insurance
Smartronix provides Basic Life and AD&D to all eligible employees at no cost. All eligible employees are automatically enrolled for $100,000 Basic Life and AD&D coverage.

Supplemental Life Insurance
Eligible employees and their dependents have the option of enrolling for supplemental life insurance. Employees are eligible in $10,000 increments up to $200,000 in guarantee issue if applying within 30 days of their eligibility date. Additional life insurance (subject to evidence of insurability) may be purchased up to a maximum of $500,000. Spouses are eligible in $10,000 increments up to $30,000 in guarantee issue if applying within 30 days of the employee’s eligibility date. Additional spouse life insurance (subject to evidence of insurability) may be purchased up to a maximum of $250,000.. Dependent life coverage can be elected in increments of $2,500 up to a maximum of $10,000.. Employees pay 100% of the premium cost for supplemental life coverage.

Flexible Spending and Transportation Spending Accounts Program
Flexible spending accounts are offered through PlayFlex. Employees may contribute up to $3,500 to a Health Care Spending Account, and may contribute up to $5,000 to a Dependent Care Spending Account. All participants in the Health Care Spending Account receive a Flex Debit card that will allow point-of-service access to their account. The Transportation Spending Account offers reimbursement for transit passes and parking expenses using pre-tax employee contributions. By using this benefit, participants can contribute up to $230 per month in 2010 for commuting expenses.

Employee Assistance Program
Smartronix automatically enrolls all eligible employees in the company-paid employee assistance program (EAP). Available benefits include an anonymous counseling line, life coaching, face-to-face counseling sessions, and website resources for such topics as marriage and relationships, parenting, legal & financial, and work to life assistance.

Concierge Program
Smartronix employees have access to LifeWorks Concierge Services. This program is offered to help employees manage time-consuming tasks easily and conveniently. Life Works can help you: find and schedule a professional housecleaner or repair service, plan and arrange a weekend getaway or a family vacation, find a house or pet sitter to look after things at home, locate and purchase concert, sports, and theater tickets or make restaurant reservations, or even help plan a party or social event.

Holiday Leave
Smartronix observes ten (10) paid federal holidays each year: New Years Day, Martin Luther King Day, Presidents Day, Memorial Day, Independence Day, Labor Day, Columbus Day, Veterans Day, Thanksgiving Day, and Christmas Day.

Paid Vacation Leave
All full-time employees accrue 15 days of vacation leave each calendar year. After 5 years of service, full-time employees accrue 20 days of vacation leave each calendar year.

Paid Sick Leave
All full-time employees accrue 5 days of sick leave each calendar year.

Smartronix 401(k) Plan
All eligible employees may join the 401(k) plan immediately upon hire. An employee may contribute up to the IRS maximum limit each year. Smartronix matches 50% of the first 6% contributed by the employee per payroll. Employer match is subject to a 5-year vesting schedule.

Profit Sharing Plan
Smartronix offers an annual discretionary profit sharing plan for all eligible employees. Profit Sharing is subject to a 5 year vesting schedule.

Formal Education Assistance
All eligible employees may receive up to $2,500 in annual tuition reimbursement for career related college courses.

Professional Development Assistance
Smartronix encourages all employees to seek professional training to continue expanding their knowledge and skills. Requests are approved based on job relatedness, business needs, and other factors.

Professional Organization Memberships
Smartronix encourages employees to seek and join professional organizations and will reimburse up to $100 annually.

Employee Referral Bonus
Smartronix encourages its employees to seek out and refer candidates to us. Referral bonuses are paid in the following amounts: $750 for full-time referrals and $250 for part-time referrals. Referral bonuses are paid once the hired employee has completed their initial 90 days of employment with the company.
